The condition is characterised by the thickening and tightening of the capsule around the shoulder joint, leading to decreased mobility and range of movement.

One treatment option for frozen shoulder is hydrodistension, also known are capsular distension, distension arthrogram and high volume distension. Hydrodistension is a procedure that uses a large volume of sterile water to stretch the capsule around the shoulder joint. The procedure is performed under ultrasound guidance and local anaesthesia, and the water is slowly injected into the capsule through a small needle. A small amount of steroid is also added to help reduce inflammation.

The pressure of the water stretches the capsule, which helps to break up the adhesions and scar tissue that are causing the stiffness and pain. The procedure can also help to increase blood flow to the area, which can promote healing and reduce inflammation.

Hydrodistension is a relatively quick procedure, typically taking about 10-20 minutes to complete, however for 60 minutes is allowed to conduct a full assessment and scan prior to the procedure. The procedure is performed as an outpatient procedure, meaning that you can go home the same day. After the procedure, patients will be given specific exercises to do at home to help regain their range of movement and strength.

One of the benefits of hydrodistension is that it does not require any incisions or cuts, just a small injection needle. This makes it a great option for patients who may not be candidates for more invasive procedures such as surgery.

Another benefit of hydrodistension is that it has been shown to be effective in reducing pain and stiffness in the shoulder and improving range of movement. In a study of 100 patients with frozen shoulder, 93% of patients reported significant improvement in their pain and range of movement after hydrodistension.

It’s important to keep in mind that hydrodistension is not suitable for everyone. Patients with certain medical conditions, such as blood clots or heart conditions, may not be candidates for the procedure. It’s important to discuss with our injecting clinicians if hydrodistension is the right option for you.

In conclusion, hydrodistension is an effective injection treatment option for patients with frozen shoulder. The procedure can help to reduce pain and stiffness and improve range of movement, allowing patients to return to their normal activities and improve their quality of life. If you are suffering from frozen shoulder, discuss hydrodistension with us today as a potential treatment option.

The start of the golf season will be here before you know it. Now is the time to start thinking about implementing physical strategies that can help you have a successful year.

Perhaps due to playing and practising less over the winter, you will have some more time to work on other physical aspects of golf. It would be useful to consider using this time to work on targeted exercise.

As you age you might find that you get stiffer and more restricted in your movement. Golf is a rotation-based activity. Most of the rotation in our golf swing comes through out hips and thoracic (middle part) spine. Keeping these areas mobile and flexible is key to maintaining a fluid swing. Doing simple mobility exercises a few times per week is enough to be effective.

There is a growing trend in the game towards distance and club head speed. A key component in generating good distance and speed is having adequate strength in our legs and ‘core’. A strengthening programme that targets these areas, done twice a week can see impressive improvements in 6 weeks. There is still time to see gains before the season kicks off!

Being mobile and string in the right areas can also make our bodies more robust in general and reduce the risk of picking up niggles and injuries during the season.

In the clinic we tend to see an increase in golf related injuries in the 1st couple of months after the season starts. This is typically due to overuse. The most common reason people get injured when returning to any sport or activity they haven’t done in a while is because of doing ‘too much, too soon’. There are ways we can mitigate that risk in golf;

Space out your return to play over a few weeks, build up how much you play gradually.

Continue to play and practice through the winter. This helps the body to avoid becoming ‘unaccustomed’ to the demands of golf.

As mentioned above, try to integrate a strength and mobility programme into your week.

Try to maintain general fitness. Don’t underestimate the fitness required to walk around a course for 4 hours and the impact that fatigue can have on performance and injury risk. If not playing regularly through the winter then try to stay fit with activity such as walking or jogging.

At Life Fit Wellness we have a number of services that can help you with your golf game, from Physiotherapy to address any injuries you have, and Pilates to help with golf conditioning, to Golf Screening to help identify physical impairments affecting your swing.

For further information about our services then please contact us at 01324 614044 or info@lifefitwellness.co.uk

What is Tennis Elbow?

Tennis elbow is the term given to pain on the outside of your elbow. It is due to injury or inflammation of the tendons that attach onto the bony bit on the outside of your elbow. Pain on the inside of your elbow is known as golfers’ elbow.

Tennis Elbow Symptoms

Some of the most common things we hear from people with tennis elbow are: pain on the outside of the elbow, pain into the forearm, pain when gripping objects, pain when using a kettle or screwdriver, pain when typing.

Tennis Elbow Causes

Tennis elbow is most commonly caused by overuse. The tendons that cause pain are part of the muscle that go into your hand and wrist. So, activities that involve of lot of gripping, lifting, typing can cause pain on the outside of you elbow. It can also occur through other means such as falls or knocks to your elbow, or through having other underlying conditions such as rheumatoid arthritis or spondylarthritis.

Tennis Elbow Treatment

There are multiple different treatment options for tennis elbow. Below, we will go through what is involved in each of them.

Wait and see

Simply put, ride it out. Adjust your activities slightly to cause less pain but otherwise continue as normal. After a year most people will be pain free.

Physiotherapy management

Physiotherapy includes a mixture of hands on (mobilisations with movements) and rehab exercises. At 6 weeks your chance of recovery is more than double that of those that wait and see. Meaning you get out of pain significantly quicker compared to wait and see

Corticosteroid injection

Cortisone injections work by reducing the inflammation in the injected area. Studies have shown a significant decrease in pain within the first 6weeks following a corticosteroid injection, with 70-80% reporting their pain had gone away or improved significantly. However, there is a higher chance of the pain returning within a year with 72% of people having their pain return compared to 8% of those that carry out physiotherapy only. But there are other injection options available that do not have the same recurrence rates. These are talked about below.

Hyaluronic acid injections (OSTENIL® TENDON)

The brand of hyaluronic acid injection we use is OSTENIL® TENDON. These injections provide relief from tennis elbow by providing hyaluronic acid at the site of irritation to facilitate lubrication. It also helps to block pain receptors and facilitates local tendon nutrients, while also reducing inflammation. There are significantly less side effects of hyaluronic acid injections compared to steroid injections.

PRP (platelet rich plasma) injections

Platelet rich plasma injections works by using the body’s own natural healing processes. The procedure involves taking a small sample of your own blood in a special syringe which is then spun at high speed in a centrifuge which separates the blood plasma from red blood cells. The plasma layer is rich in platelets which release growth factors that promote repair within affected tissues. The plasma is then drawn into a separate syringe using a clever system which then allows it to be injected into the tendons causing tennis elbow.

Shockwave therapy

Shockwave therapy is delivered using a machine that delivers high-energy sound waves to the tendons of the elbow. These sound waves cause an increase in blood flow, which stimulates the body’s healing response. This is delivered alongside physiotherapy and helps to speed up recovery further.

If you would like any additional information, then please contact the clinic or visit our relevant Tennis Elbow condition and injury advice section on our website.

This is based on the TPI (Titleist Performance Institute) screening process but with our own unique additions. Emmet, who runs this service, is TPI certified. This is a world renowned qualification that provides a useful structure to assess how a golfer moves and to guide development of suitable rehabilitation and conditioning programmes.

There are 2 main reasons why you might seek out a golf screening session:

The first reason is to try to identify physical impairments that may influence your golf swing. You might be working with a golf professional who is trying to get you to swing the club in new way but you are struggling to do so. A screening assessment will be able to tell us if you have the physical capabilities to swing in the way your professional is trying to coach you to do so. If you have some restrictions then we will be able to get an insight into whether they are improvable or not. In other words, answering the question of should you work on an exercise programme to make improvements so you can swing in a more ‘optimal’ way or do you build a swing to fit around the restrictions that you have.

The other reason a golf screen might be useful is in the context of injury management. If you are injured or recovering from injury, an assessment will give us information about the way you move and the possible role this has on your pain. This can help us to guide your recovery via targeted rehabilitation. An injured golfer is an unhappy golfer so anything that helps to optimise their return to full play should be emphasised. Of useful note is that screening in general is not thought to be very helpful in predicting injury. So if you are currently injury free, a golf screen in itself won’t necessarily tell us how likely you are of picking up a future injury.

If you would like any additional information then please contact the clinic on 01324 614044, email Emmet at emmet.kenned@lifefitphysiotherapy.co.uk or visit our relevant Golf Screening section on our website.

Ask some golfers what Pilates is and it may conjure up comments of – let’s face it -mainly women on mats trying to work their elusive “core”.

But ask those trained in Pilates about the benefits it can bring to sport – and in particular golf – they would say they are plentiful.

The biomechanics of the golf swing are complicated involving proper co-ordination of muscles, mobility, and force production. Almost all parts of the body are utilised in an efficient swing and having the optimal balance, strength and mobility in each area can be enhanced by Pilates practice.

So why should Golfers consider using Pilates to enhance their performance and enjoyment of the sport?

1. Spinal Mobility

Flexibility between shoulder and hip is paramount to an effective and efficient swing. The spine rotates multiple times during a round of golf. Pilates exercises can enhance spinal rotational as well as target shoulder and hip mobility deficits.

2. Core Strength

This is not just about your abs!

Your “core” is the muscles that act around your centre to stabilise your spine including your back muscles and deeper muscles of the spine. By improving the activation, strength and endurance of these muscles it can help force production from ground, to club, to ball.

Having a stronger, more resilient core may also help improve your golf posture, and reduce injuries to the spine.

3. Balance and Stability

One of the key elements of Pilates practice is correct alignment. Muscle imbalances, previous injuries and reduced general mobility can make proper alignment challenging.

We work on both stability AND flexibility within class to help ensure alignment is optimal.

At our Life Fit classes we also utilise small pieces of equipment to challenge muscles further to increase the benefit of each exercise, this also keeps things interesting from week to week! From poles to weights to magic circles we like to utilise different props to enhance both mobility and strength.

We also have the option of our Large Equipment Combo classes which add a different dynamic and challenge to the classical Pilates movements.

4. Mind/Body Connection

Both golf and Pilates require your mind to be connected to your body. The complexity of the swing means the more mindful you are with your movement the more effectively signals can be transmitted to each muscle group. In Pilates practice this is a concept we use with every exercise – strengthening the mind/body connection and thus creating good movement habits.

5. Injury Prevention

Golf is a very asymmetric sport which can lead to muscle imbalances in the lower limb and spine. These may lead to muscle tightness and potentially injury due to this. Pilates can help to address these imbalances and work your body as a whole. You may also become aware of these weaknesses when practicing which can allow you to target them more effectively before they lead to issues.

Prevention is always better than cure!

And what happens when you suffer with embarrassing problems related to your ‘normal’ birth? Forceps deliveries, tears (including third and fourth degree tears that tear into the anal sphincters), prolapse and incontinence can all occur after giving birth. And contrary to what the marketing people at a certain incontinence pad company would have you believe, ‘Oops moments’ are not normal, or even worse, that they should be accepted as part of your life from now on.

There is still a very ‘fairy tale vision’ of childbirth, where in reality many women suffer significant injuries or issues following it. Research shows that one in five women develop problems with bowel control in the first five years following vaginal delivery, and there are many more who have prolapse and urinary incontinence. Women are too embarrassed to draw attention to their pelvic floor issues after childbirth, many feel shame, or even fear of what may happen during examinations, or feel that surgery is their only option.
Women should be offered a 6 week check by their GP following birth. However, many have missed out on this due to pressures on the services during Covid. There are pelvic health physiotherapy departments in most major hospitals but again waiting times can be long and some GPs are still unaware that these even exist therefore women are just not able to access the care they require.

Kate, our pelvic health physiotherapist, has seen many women throughout the past 2 years who were seen by their GP/hospital and diagnosed with a prolapse and then basically shown the door. No information, no reassurance and no advice on how to move forward with their rehabilitation. A pelvic health physiotherapist can carry out a full pelvic assessment including internal examinations and diagnosis of prolapse. For many, conservative management is all that is needed. Physiotherapists can provide most of this, including pessary fitting if required.

So where is the help when you need it?

Thankfully help is available at Life Fit Wellness in our pelvic health clinic with our specialist pelvic health physiotherapist Kate Leishman.
Walk whatever the weather. Don’t wait until it is ‘perfect’ or you simply won’t go. Puddles are great to discover your inner child again. Do be mindful of icy conditions: avoid early in the morning or in poor light, stick to main routes that are more likely to be gritted or get into a park on grass if possible.

Mix it up – explore the streets in your neighbourhood. Some might be new thanks to recent development, or some just new to you!

Check out your neighbourhood. Ok this is bit of a confession. I look at fences (we need new ones and I was getting ideas), also gardens, the types of pots and plants. And to those of you who put your lights on and don’t pull your curtains, it is great to get decorating ideas from your lit up rooms as I pass by!

Record your daily walk perhaps in a diary, using an app like Strava, taking photos of where you have been or collecting something en route (if I’ve been at the foreshore I’m bound to have collected at least one pebble!)

Prepare for your walk with some research on local history, local landmarks. I’ve taken the opportunity to find out more about the history of Bo’ness then walked to discover them. Some familiar, some off the beaten track…Witches Stone anyone?

Another way to mix it up is vary your pace. Maybe a short, fast walk when time is precious or a long, leisurely one if you have time to spare. Believe me 30mins of up/down hills in Bo’ness is tough!

Pack a picnic in your rucksack (flask of soup or hot chocolate on these cold winter days) walk and then sit somewhere local and just enjoy scenery that you probably see every day but don’t take time to appreciate.

Use an app like World Walking which lets you choose a route and walk it virtually. It gives you milestones and lets you know of landmarks you’ve passes on your way. In the first lockdown we explored the castles of Scotland on foot…the only one I physically visited in that time was Blackness.

Take a sensory walk. Instead of being plugged in and tuned out use your senses, listen, look and smell. Warning: you won’t want to ‘smell’ if your route takes you on the John Muir way past the sewage works in Bo’ness!

Ask yourself does all of your work have to be done indoors or do you just need to reframe how and where you do it? Can you make work calls whilst out walking, or perhaps listen to podcasts, webinars, use the record function on phone to dictate notes to yourself.

Apply this to home schooling too for those of you who are having the additional stress of fitting that into a daily schedule. On one of my walks I watched a mum out with kids who had chalk with them and they stopped to do a sum/write a word on the pavement. What a fantastic idea. Whilst you are outside consider geography, history, PE as subjects easily covered! Your topics might not be on the curriculum but it’s all learning, right?

This was going to be a list of 5 but once I got started and really thought about what I have been doing I realised I have been more inventive than I was giving myself credit for. I have certainly have re-framed my lockdown limitations to liberations and hope you can too. Watch out for our WalkFitWellness photos of local walks putting some of the above ideas into practise (beware if you have your light on and curtains open…you might feature as decorating inspiration!)

I wish we had picked a better time given all the changes and additional stress from the pandemic. If you see me, please don’t ask if I have started packing or when I’m going! At the time of writing this, I still have no visa or flight booked, so slightly anxious about it all. I’m keeping all my fingers and toes crossed that it all goes to plan (I can actually do that because of all the yoga & Pilates I’ve been teaching). If all goes well, we’ll be moving to Hawkes Bay in the North Island which is where much of the county’s wine is made, so an extra incentive to get there!

I wanted to reflect on my time here at Life Fit since March 2017. I was not sure how to present this and really didn’t want it to sound like a eulogy or a CV. I settled on compilation of highs and lows, interesting random facts and lots of photos to keep it fun.

Chartered Physiotherapist Emmet Kennedy shares some thoughts relating to his own exercise.

One of the key parts of our profession is to encourage people to do exercise, both in terms of rehab and for general health benefits. As a physiotherapist I feel it is important to try to lead by example and to practice what I preach. Adherence to rehab exercise is actually quite poor and the number of people of who don’t meet the recommended guidelines for physical activity is much higher than it should be. We all know that being active is important for loads of different reasons, but there are lots of things that get in the way to prevent us from hitting the goals we set ourselves. I thought it might be of interest to share some personal insight into how I fit exercise into my own week.

I work full time at the clinic – Monday to Friday, including 2 evenings per week so that automatically limits my available time to exercise. Life is also really busy away from the clinic. My main focus just now is my young family. My wife, Kirsty, and I have a very energetic 2 year old at home, called Shay, who keeps us on our toes! Shay was recently joined by his little sister Orla, who is now 9 weeks old (time is flying by!).I get home from work on normal days at about 5.30pm. Before we had kids I tended to come home, get changed and go straight back out to the gym. Shay goes to bed at about 7.30 in the evening so those couple of hours when I get in are now prime family time. Spending time having dinner, bathing the kids, reading stories etc. are a good way for me to unwind from work. Once Shay is in bed (fortunately he’s a good sleeper) our routine is to tidy up the enormous mess he has made (!) and to do some general household tasks, which takes us to after 8 at some point. Orla is then usually feeding or sleeping around this time so it gives me an opportunity to go exercise. The thought of going back out is one I often can’t be bothered with. Getting up off the couch is pretty difficult and I often feel like just nodding off for a sleep. However, I force myself up, have a quick shot of espresso to perk me up and then go do some exercise. I’ll either go for a run or do some weights at the gym (luckily my gym is open quite late), which I’ll manage to do 4 or 5 times per week. I’m normally out for 45 minutes and come back feeling fatigued but energised. I then often sleep much better on the nights that I’ve exercised, apart from when Orla is up to feed!

By and large I have managed to be consistent with my exercise over the past few months and it is now part of my normal week. Getting to this point definitely makes it easier for me to maintain. Like a lot of people I also use exercise to help regulate my mood and it’s much easier for me to manage the stress of life when I’m regularly active. It’s challenging to fit exercise into the week and would be easy to let it slide but finding the time to do so makes the rest of my week better.
